CARE CO-ORDINATOR Harrow Salary: £27,212.02 – £29,862.13 per annum + London Allowance Permanent, 37.5 hours per week | Part-time hours considered We are currently recruiting for Care Co-ordinators on behalf of a well-established specialist healthcare organisation providing 24-hour specialist care to patients and families across North West London. This is an exciting opportunity for an organised, compassionate and proactive healthcare professional to join a caring and diverse multidisciplinary team and play a vital role in coordinating high-quality, person-centred care. You will work alongside senior nurses and a wide range of health and social care professionals to ensure patients receive timely, coordinated support and, wherever possible, remain in their preferred place of care. About the Role As a Care Co-ordinator, your responsibilities will include: Coordinating referrals and supporting patient care pathways • Working closely with the nursing team to triage and prioritise referrals • Acting as a key point of contact for patients, families and healthcare professionals • Supporting a 24-hour advice and support service • Facilitating effective communication between healthcare providers • Monitoring referrals and maintaining accurate records • Providing information, guidance and signposting to appropriate services • Supporting efforts to prevent avoidable hospital admissions • Working collaboratively with NHS 111, ambulance clinicians, GPs, community services and hospice teams • Contributing to a compassionate, responsive and high-quality palliative care service About You We are looking for candidates who are: ✔ Compassionate, professional and patient-focused ✔ Highly organised with excellent attention to detail ✔ Confident communicating with patients, families and healthcare professionals ✔ Able to work effectively in a fast-paced healthcare environment ✔ Comfortable working as part of a multidisciplinary team ✔ Proactive and able to prioritise competing demands ✔ Flexible to work as part of a 24-hour, 7-day service, including potential night shifts ✔ Experienced within healthcare coordination or a relevant healthcare setting Location The primary base is Harrow with regular travel to partner healthcare sites across the area in line with service requirements
